New Patient Appointments

We Welcome New Patients!

Welcome to Durham Pediatric and Orthodontics! We’re honored you are entrusting us with your child’s dental care and look forward to being part of your “health” family for many years. New Patient Appointments are usually available within a few days of your initial call. Our office staff will work with you to find convenient appointment times and we will do our best to minimize appointments that interrupt the school or work day. 

 

Before Your First Visit:

New Pediatric Dental Patients:

We look forward to meeting you and your child at your first appointment. Please print out and complete a copy of our Pediatric Health History Form, and bring it with you to your first appointment. You may mail or FAX the form to us. A copy of our Privacy Policy is also available for you to review (HIPAA).

If your child has been seen by another dentist, please contact that office at least two weeks in advance and request that your child’s x-rays and dental history be sent to us. This will provide us with important dental information and also avoid unnecessary duplication of x-rays for your child.

If you have dental insurance, please provide that information when you are scheduling your appointment. Always bring your current insurance card with you to each appointment. Please refer to our Insurance page if you have additional insurance questions.

New Orthodontic Patients:  

We look forward to meeting you to talk about orthodontic care for you or your child. Please print, complete a copy of our Orthodontic Health History Form, and bring it with you to your first appointment. You may mail or FAX the form to us. A copy of our Privacy Policy is also available for you to review (HIPAA).

Please let us know if your child’s dentist has referred you to our office for orthodontics. Dr. Christensen will correspond with your family dentist and keep them updated on your child’s orthodontic progress. If your child has had x-rays taken at another office, please contact that office at least two weeks in advance and request that their x-rays be sent to us. This will not only provide us with important information but could also avoid duplication of x-rays for your child and help minimize your costs.

If you have dental insurance, please provide that information when you are scheduling your appointment and bring your current insurance card with you to each visit.

Rescheduling Appointments

If you need to reschedule or cancel an appointment, please call and give our office as soon as possible. We understand schedules change and we would like to offer your time to another child who needs care. Cancellations or missed appointments without 24-hour notice are subject to a $25 fee (per child).

 

Staying on Schedule

We know your time is valuable and we do everything we can to stay “on schedule.” Sometimes there are dental emergencies or unexpected patient conditions that cause us to run behind schedule. If this happens on your appointment day, out of respect for your time, we will talk with you personally and let you decide if you want to wait or reschedule the appointment.

As a patient, there may be occasions when you are not on time. If you are running late, we will decide when you arrive if we can complete your treatment in the remaining, allotted time. We do this out of courtesy for the patients scheduled after you.
